Nutritional Support Coordinator (Remote – US)
 
Compensation: $19.00 – $23.00/hour (based on experience). 
Schedule: Monday to Friday, 8:30 AM – 5:00 PM (within your time zone). 
Location: Remote (must reside in the U.S.). 
Work Environment: This position is remote; however, if you reside in or near Tampa, FL or Independence, OH, it will be offered as a fully on-site role with in-office expectations.

This is a high-volume, metrics-driven role ideal for nutrition professionals who thrive in a fast-paced, structured environment and enjoy building patient relationships through daily outreach and accountability for performance.

Key Responsibilities: 

  • Manage a panel of patients and complete required monthly outreach. 
  • Conduct daily phone calls with patients, providing care plan support and health coaching. 
  • Document all patient interactions in real time using our care coordination platform. 
  • Collaborate with care teams, providers, and caregivers to address clinical or social needs. 
  • Maintain call quality, HIPAA compliance, and performance standards on every interaction. 
 

Performance Metrics: 
To be successful in this role, Nutritional Support Coordinators must consistently meet the following expectations: 
  • Daily Outreach: Calls are expected consistently from 8:30 AM to 5:00 PM, excluding breaks and lunch. 
  • Patient Load: Actively manage a caseload of 300–350 patients. 
  • Monthly Touchpoints: Stay on schedule with all monthly outreach goals; No patient left behind. 
  • Time Tracked: Use our real-time ticker to record your full workday, ensuring all patient-facing, care plan activities, and related activities are accurately captured. 
  • Quality & Compliance: Pass regular QA call reviews, maintain HIPAA standards, and ensure accurate, real-time documentation.

Required Qualifications: 
  • A Degree in Dietetics or Nutrition.
  • 2+ years of experience in clinical nutrition counseling or dietary care coordination.
  • Experience supporting patients with chronic conditions in in-home, in-facility, or remote settings.
  • 2+ years of EMR and computer navigation experience.
  • Demonstrated ability to educate patients, simplify complex nutritional concepts, and support weight management goals.
  • Must reside in the United States, within Eastern or Central Time Zones.
  • High-speed internet (minimum 50 Mbps download / 5 Mbps upload) and a HIPAA-compliant, private home office.
 

Preferred Qualifications: 
  • 3+ years of experience in healthcare nutrition, chronic care management, or patient education.
  • Certification as a:
    • Registered Dietetic Technician (NDTR), or
    • Certified Nutritionist, or
    • Certified Health Coach
  • Bilingual (Spanish/English) preferred.
 

Compensation & Benefits: 
  • Hourly Pay: $19.00 – $23.00/hour, based on experience. 
  • Incentive Pay: Lift Pay (individual productivity bonuses).

Work Environment & Requirements: 
  • 100% Remote with a Monday–Friday schedule—no weekends or late nights. 
    • Work hours: 8:30AM - 5:00PM (within your time zone). 
    • If you reside in or near Tampa, FL or Independence, OH, the role will be offered as a fully on-site position with in-office expectations. 
  • Company-provided equipment (Mini PC, monitor, headset, etc.) 
  • High-speed broadband internet and private home workspace required 
    • Candidates must complete a company-provided internet speed test to confirm a minimum of 50 Mbps download / 5 Mbps upload. 
  • Private workspace for compliance with HIPAA privacy laws.
