What You’ll Do:
- Provide one-on-one telehealth coaching to assess patients’ dietary needs and health goals.
- Educate patients on meal planning, healthy eating habits, and weight management strategies.
- Serve as an advocate for patients’ nutritional needs by actively listening, addressing concerns, and collaborating with their healthcare team.
- Develop personalized nutrition care plans and monitor progress over time.
- Communicate professionally with care teams to ensure a seamless patient experience.
- Maintain accurate documentation of all patient interactions.
What We’re Looking For:
- A dedicated, empathetic professional with strong active listening and communication skills.
- Ability to work both independently and as part of a collaborative healthcare team.
- Bilingual (Spanish/English) candidates are a plus!
- Comfortable adapting to new processes and technology-driven healthcare solutions.
- Medical specialties: gastroenterology, geriatrics, home health, internal medicine, primary care, and urology.
Required Qualifications:
- 2+ years of experience in clinical nutritional counseling, or dietetics care coordination.
- Experience in adult in-home, in-facility, or remote chronic conditions care management addressing nutritional needs.
- Ability to translate medical information, instructions, and documents to ensure comprehensive understanding.
- Demonstrated knowledge of nutrition education, coaching, and fundamental dietary principles.
- Previous experience in weight management.
- 2+ years of computer skills and EMR experience.
Preferred Qualifications:
- 3+ years of experience in healthcare nutrition, chronic care management, and patient education.
- Degree in Dietetics or Nutrition preferred.
- Certifications:
- Registered Dietetic Technician, or
- Certified Nutritionist, or
- Certified Health Coach
Compensation & Benefits:
- Competitive hourly rate: $19.00 - $23.00 per hour, based on experience.
